Job Summary:

Provides basic nursing care and assistance with activities of daily living to patients in their homes.

Essential Duties and Responsibilities:

- Assists with personal care tasks such as bathing, dressing, grooming, toileting, and feeding.
- Reports changes to the RN.
- Assists with ambulation and transfers, using proper body mechanics and assistive devices as needed.
- Provides light housekeeping tasks such as meal preparation, laundry, and changing bed linens.
- Reminds patients to take medications as prescribed.
- Documents care provided and observations in patient records.
- Communicates effectively with patients, families, and the healthcare team.
- Adheres to all agency policies and procedures and maintains patient confidentiality.

Qualifications:

- Current CNA certification in the state of Massachusetts.

- High school diploma or equivalent.
- Experience in home health care preferred.
- Reliable transportation and valid driver's license.
- Compassionate and patient-centered approach to care.
